2567: a dull keratome is not worth the risk

Close-up of an eye during surgery showing an orange object on the cornea, with text warning against using dull keratomes and highlighting the difficulty in penetrating tissues.

Today we are featuring a resident case where the video is shown at 2x speed since the surgical time is 20 minutes. The resident starts out using a dull keratome and that makes it challenging to perform the incision. If you try to overcome that with additional force, you could get into trouble with the blade suddenly entering the eye and causing iatrogenic damage. A much better option is to open another, fresh blade. This resident also struggles with some basics such as pivoting / floating within the incision without distorting the cornea as well as keeping the eye in primary during the procedure. Please comment below with your best advice for this young surgeon.
