This patient had a prior pterygium surgery which did not go so well. It resulted in a corneal perforation and then that required surgical repair. That is why you see a suture in the central cornea and conjunctival overgrowth on the cornea. In addition notice that there is corectopia with the nasal iris likely plugging up a corneal perforation site. This patient now also has a cataract and wishes to improve the vision somewhat.
