This patient has vitreous hemorrhage and a cataract and will undergo combined phaco, IOL implantation, and pars plana vitrectomy. The challenge in the cataract portion of the surgery is visualizing without a red reflex. We can use trypan blue dye to stain the anterior lens capsule but how do we see well enough to thoroughly remove the lens cortex? Our guest surgeon uses the light pipe for endo-illumination and completes the case beautifully. Most of our CataractCoach fans also have an interest in cases with retina pathology and for that we are launching our sister channel RetinaRounds.com in two days, on March 1, 2025.
