2488: trying to remove cortex but then this

Looking at the title picture, we all feel a little pain. This happens to all surgeons at some point: we are trying to remove cortex but then the suction grabs the capsule and then the zonular loss happens suddenly. This can be a combination of iatrogenic causes but also due to the patient’s pre-existing zonulopathy or tissue pathology. In this case, what is your next move? How will you secure the IOL? Is there vitreous prolapse? What are your options?
