This patient has a small eye with a small pupil, shallow anterior chamber, and pseudo-exfoliation. That will make the case quite challenging. Our guest surgeon uses a pupil ring to provide mechanical mydriasis and to keep the iris out of the way. Another challenging aspect is that our surgeon is sitting superior but making a temporal main incision. This means the surgeon’s right hand holds the phaco probe when operating on right eyes while the left hand will be used when operating on left eyes. Great case from a talented surgeon. Have you tried this seating position when operating?
