This patient had cataract surgery done many years ago and everything was great until recently. The patient now presents to you with severe progressive zonulopathy and a dislocated IOL and capsular bag complex. The original surgeon many years ago did a great job and the issue here is the patient’s tissues and underlying disease process. Pseudo-exfoliation (PXF) is a progressive disease and as these patients live longer and longer, the incidence of late IOL+bag dislocation increases. Getting old is not for wimps!
