2471: Learn the ominous Snap Sign!

Professor Ronald Yeoh from Singapore taught us all about the Snap Sign many years ago. The “snap” is when the pupil suddenly constricts (in less than a second) as balanced salt solution from hydrodissection ruptures the posterior capsule. This is more common in cases such as posterior polar cataracts and posterior capsule damage from prior vitrectomy or intra-vitreal injection. But it can also happen when there is a small capsulorhexis and the cannula prevents the nucleus from moving anterior to release trapped balanced salt solution.
