2465: approach to short axial lengths

I enjoy operating on very myopic eyes because there is so much room within the anterior segment of the eye. The very small, hyperopic eyes are not as simple because the anterior chamber is shallow and there is much less room to maneuver. Cup your hand and in your palm place 5 drops of water. That’s the volume of this anterior chamber (<250 microliters) and we have to work within that tiny space! There are many considerations when operating on eyes with short axial lengths which border on being nanophthalmic. This video explains the challenges and shows you my approach to these cases.
