This patient has a rare condition called ICE syndrome which is irido-corneal endothelial syndrome which affects unilaterally. This iris is irregular with polycoria, atrophy, and an endothelial membrane spreading within the anterior chamber. The patient needs to have cataract surgery and then an artificial iris implant to reconstruct the anterior segment and provide improved vision as well as a better cosmetic result to match the other eye.
