
This is a very interesting case where the surgeon tries to puncture the anterior lens capsule but it is very difficult even with a sharp needle. Finally the surgeon is able to enter the capsular bag and decompress it. But then there seems to be no nucleus. So the question is: Where did the nucleus go? The nucleus is posterior because this cataract was likely caused by previous VR surgery where the posterior capsule was violated. It is most likely sitting on the retina. I have seen this happen.
