2235: Cataract Quiz: What are these lines?

This is a routine case and there is a nice incision, a great capsulorhexis, and the cataract was removed efficiently. Now the capsular bag is full of viscoelastic and the IOL is inserted carefully. As the IOL unfolds, you notice some faint lines. What are these lines? What should you be concerned about? What is your next move?

  1. What is the first IOL had been left in place overnight and the result seen the next day? The extra manoeuvres led to the PCR, vitreous prolapse, having to inject Triamcinolone which went into the vitreous cavity and could cause IOP spikes. More endothelial cells must have been lost; overall leading to delay in visual recovery and the most dreaded of all; inviting infection and inflammation in an otherwise excellently done surgery.

  2. The forceful injection by the automatic injecting system led to the problem here. The original lens could have been repositioned in the bag away from the defect in the capsule and left in the bag.
