2228: you notice this IOL optic crack…

In this case, the patient started with a dense, opaque cataract and a pre-operative vision of 20/200 or worse. The cataract surgery goes beautifully but then challenges arise when the IOL insertion is performed. The incision is too tight and the IOL gets stuck in the incision. Once the IOL is finally forced through the incision, you notice this crack in the center of the optic. What is your next move? Can you leave it like this? Or should you perform an IOL exchange?
