2117: capsulorhexis with the bimanual IA

Our guest surgeon uses the bimanual irrigation/aspiration instruments to perform the capsulorhexis in an intumescent white cataract. The anterior capsule is held by the suction of the aspiration probe while the irrigation probe gives the infusion pressure to stabilize the anterior chamber. Have you tried something like this?   1. I don’t like this personally because you need to keep the chamber pressurized with viscoelastic and you can’t do that with an I/A probe on suction. This makes little sense to me. I prefer to fill the AC with viscoelastic and debulk the lens cortex with a 25 g needle and then make a controlled tear.

  2. Ot looks the surgeon did not expect fluid to come out; the choice to stay in after milk aspiration could be to avoid repeated AC decompressions: it seems very smart and skill to me.
