2084: modified twist and out IOL explant

We have featured the twist-and-out technique to explant an acrylic IOL through a standard phaco incision using just a spatula and tying forceps. Our guest surgeon, Dr Morgan Micheletti made a modification to the technique by placing a 25g cannula across the anterior chamber via two opposite paracenteses incisions. This allows him to then use two hands to twist the forceps to roll up the IOL for extraction. He is a very innovative thinker and we discuss that in detail in his CataractCoach PodCast which goes live tomorrow. We had a great discussion and he shares some important insights for success for early career ophthalmologists.     1. yes, works for acrylic 3-piece IOLs, just be careful with the trailing haptic. For silicone 3-piece IOLs, they are very slippery when coated with viscoelastic and this technique can be much more challenging
