
This patient experienced a dropped nucleus during cataract surgery with extensive damage to the capsular bag. This poses a challenge for IOL fixation with the options being an anterior chamber IOL (ACIOL), an iris-clip IOL (not available in USA), fixation of a three-piece IOL to the sclera (Yamane, Glued IOL), or a sutured IOL (Canabrava or Gore-Tex). But there is also another option: this newer design IOL that has anchor style haptics. Each of these haptics has a T-shape which is designed to be passed through the scleral wall and then secured into a scleral pocket. There may be other similar designs as well, though I doubt we will have these FDA approved and available in the USA anytime soon. Great learning case none the less.
