
This is a tough case where the surgeons learns intra-operatively that there is a quadrant of poor zonular support. An Ahmed capsular tension segment is placed in the eye to provide support during nucleus and cortex removal and then it is carefully sutured to the sclera. There is also a placement of a full capsular tension ring (CTR) to ensure total capsular support. This case is very nicely managed and the patient achieved a great result. Lots of great pearls to learn from this video for when you encounter the same situation. Patients don’t always recall previous episodes of ocular trauma and it is very difficult to detect this type of quadrant zonular weakness at the pre-operative consultation.
