
Another tough case today: as this resident implants a three-piece IOL, the leading haptic is twisted and the sharp tip of it tears the posterior capsule! Always remember the 7L rule when inserting an IOL, especially a three-piece design which tends to have more variability in delivery. The leading haptic should come out looking like the number 7 and the trailing haptic should be oriented like the letter L. Fortunately with this three-piece design, placement of the haptics in the sulcus is able to be performed and the patient achieves a good outcome.
