Look carefully at your phaco incision to see if you are making the mistake of the chevron incision. The roof and floor of your incision should be balanced and the entry into the anterior chamber should be square shaped. If the tip of your incision looks like a chevron then it likely means that you are making too abrupt of an angle change as you enter the anterior chamber. This results in an imbalanced incision that does not seal well. This video shows OCT imaging of a phaco incision with the chevron mistake and explains how you can fix your technique to provide better results for your patients.
