
The capsulorhexis has a tendency to run-out if the anterior lens capsule is insufficiently flattened, if there is any posterior pressure, and if the anterior chamber shallows. We can rescue the capsulorhexis using techniques such as the one described by Prof. Brian Little where the flap is pulled backwards in order to bring the leading tear back centrally. This video shows some of the challenges with a run-out capsulorhexis and how to rescue these cases to have a successful cataract surgery.
