We all love a great red reflex during cataract surgery because it makes is much easier to see details such as the capsule edge during capsulorhexis creation. We can adjust our microscope lighting in order to emphasize the coaxial illumination which will improve the red reflex. This enhanced red reflex will also make for better videos for sharing with colleagues. In this case, the red reflex is so good that it appears that the nucleus is relatively soft. The opposite is true and it is actually a dense cataract. This is seen when the nucleus is brought out of the capsular bag for phaco chop. This is a complete cataract case with the entire case shown from the first incision to the end of the case. It is well worth the 6 minute investment of your time.
