Quiz: Cataract & Penetrating Corneal Injury

This patient had a penetrating corneal injury many years ago. When we look at the eye we can see evidence of this in the corneal scar and an iris trans-illumination defect. There is also a gap between the iris and the anterior lens capsule in this area. What are the risks here? How would you approach this case differently?
