This surgery is being performed by a senior resident and it starts off great. It is a dense white cataract and the resident surgeon makes an excellent capsulorhexis after staining with trypan blue dye. Then a deep groove is carefully sculpted in the nucleus and then the challenge begins.

  1. Excellent case and management. I like the way he used viscoelastic that prevented the nucleus falling back and also tried to protect the endothelium.
    However I think the capsule was violated during grooving because he went too far peripheral and at some point tip occlusion had happened and we can notice an incomplete punch sign indicating post. Capsule opening.
