This patient has a relatively opaque cataract with a pre-operative vision of only counting fingers. When we go to perform hydro-dissection, it seems to go well without issues. But for some reason, the nucleus will just not rotate. We try again and again, but no luck. The zonular support and capsular status seems normal, so a decision was made to perform phaco chop within the capsular bag. Once the nucleus was removed, it became clear the reason why nucleus rotation was not possible.

Very good demonstration, I regularly follow your videos. This is a case of Cortico-capsular adhesion. Another cause of not rotation of nucleus. Usually have seen these patients have cortical cataracts with fluid pockets initially and later as the fluid absorbs, strong adhesions occur. Mostly associated with nuclear sclerosis. Here in India we do get to see it more often
