Review: Four Quadrant Chop-and-Conquer

The idea behind divide-and-conquer makes a lot of sense: divide the nucleus into four quadrants which are more easily phaco-aspirated from the eye. A significant amount of ultrasonic energy is required to sculpt the grooves, often more than is required to remove the quadrants.

Using the phaco chopper to break the cataract nucleus into four quadrants is more efficient, both in terms of ultrasonic energy as well as time. This case also features a full-thickness paired incision in order to achieve about 1 diopter of flattening for treatment of against-the-rule astigmatism.
