2886: extensive zonulopathy from trauma

Close-up view of an eye showing extensive zonulopathy due to trauma, with a focus on lens removal and securing the bag and intraocular lens (IOL). Blood is present around the eye structure, indicating surgical procedure considerations.

This is a very challenging case where the patient sustained severe trauma. In this title picture you can see at least 6 clock hours of zonular loss with a decentered crystalline lens. The patient is also going to require a vitrectomy with silicone oil so keeping the capsular bag can help with compartmentalizing the oil and preventing it from migrating into the anterior chamber. How would you complete this very difficult case? Is a capsular tension ring (CTR) enough? Or should you also plan on suturing the bag to the sclera using a Cionni ring or capsular tension segment (CTS)? Watch the video and please leave a comment as to your best approach for this case.
