2773: global zonular laxity and weakness

Close-up of an eye during cataract surgery, focusing on the lens with instruments in place. Text overlays discuss global zonular laxity and provide instructions for completing the rhexus.

Our guest surgeon, Dr Sergio Kwitko from Brazil, is superbly talented at complex anterior segment surgery and he presented this great case at a recent conference. I was so impressed that I asked him for the video to present to our CataractCoach fans. This patient has global zonular laxity and frank phacodonesis with a dense cataract. The question is how to remove the nucleus safely and then secure an IOL in this eye. When shown the case I opined that MSICS with a scleral fixated IOL would be a great option, so I was very surprised when he was able to perform phaco and secure an IOL. This is a must-see video and I’m interested to know what you would do in this case? Please comment below.
