2612: Cataract Surgery on Cataract Surgeons

Close-up view of cataract surgery showing the lens capsule and instruments used, with text highlighting the importance of a beautiful capsulorhexis.

So far I have done cataract surgery for 84 fellow ophthalmologists (this title slide says 83 since I made it last week and I have since done surgery for a very sweet cataract surgeon from Michigan with PXF who flew out to Los Angeles to have her own surgery in our Beverly Hill surgery center). I am not sure how this ended up happening, but I assure you that I give every patient (and every fellow ophthalmologist) the same high level of care that I want for my own eyes. This video is from ophthalmologist 83 who was hyperopic with a significant degree of corneal astigmatism (and he was also the tallest ophthalmologist on whom I’ve operated). If you want to see only videos of me operating (uncut from start to finish) then check out the Complete Cataract Cases series here.

The key points are:

-the vast majority of ophthalmologists (>90%) want a monofocal IOL

-our colleagues always want to address astigmatism at the same time as cataract surgery

-everyone wants a beautiful incision and capsulorhexis (your signature!)

-ophthalmologists know what they want! Just ask them

-myopic patients always want some degree of myopia. if they want plano what they mean is -0.25 to -0.50 instead. And if they start off as highly myopic, then making them -1 to -2 is the sweet spot

-hyperopes do not want to end up myopic and they would rather be +0.25 instead of -0.25

-they all say that what they experience as a patient is much different than what they thought it would be

-ophthalmologists want to see a video of their surgery (and I provide it on a USB memory stick so they can go home and watch it over and over)

-100% of ophthalmologists who have had cataract surgery with me say that their vision is so much better and that they love being pseudo-phakic

If you are an ophthalmologist who has had cataract surgery, please share your experience in the comment section below.
