This is a very difficult and challenging case where our guest surgeon does an amazing job and offers so many great pearls for us to learn. The patient has pseudo-exfoliation syndrome, a shallow anterior chamber, a small eye, and a white cataract. The decision was made to perform a limited anterior vitrectomy via the pars plana in order to help deepen the anterior chamber. This should be done very carefully and the video shares the important pearls. If you are uncomfortable placing a pars plana trocar, then you should be learning how from our sister channel RetinaRounds.com which will teach you! If you tried giving mannitol IV and the AC was still very shallow, would you be comfortable doing this pars plana tap? How far back should you place the trocar? Comment below, please.
