
With tens of millions of intra-vitreal injections being performed worldwide, a seemingly rare complication like capsule rupture happens to many patients per year. In this case the patient developed a white cataract just a few days after an intra-vitreal injection of Anti-VEGF medication was given for the treatment of diabetic retinopathy. This means a rather large compromise of the lens capsule so that the lens proteins are exposed to the immune system which elicits a strong inflammatory response and then the white cataract formation. How would you approach this case? What are the odds that there will be nuclear fragments that fall into the vitreous cavity? How will you manage that?
