2815: Why did the nucleus drop?

An anatomical image showing the vitreous cavity of an eye with a dropped nucleus, accompanied by a question about why it happened. Text overlay prompts viewers to help solve the mystery.

Let me first start out by telling you that I do not know exactly what happened here and we are counting on the crowd-source, group knowledge to solve this mystery. This patient has a cataract without any prior history of trauma. No prior surgery either. And no prior intra-vitreal injections. Then during a seemingly normal cataract surgery the nucleus drops into the vitreous cavity. Why? Of course there is an issue with the patient tissue and protoplasm, but what else? Help us solve this mystery by watching the video and commenting below. Teach me!
