Our guest surgeon uses 8-0 Gore-Tex suture to fixate an IOL with eyelets to the sclera using a transconjunctival approach so that no peritomy is needed. The suture is cleverly passed through the vitrectomy trochars and once they are removed a knot can be tied and then rotated to be inside the eye. This leaves just the smooth band of Gore-Tex suture under the conjunctiva. The alternative is to use the intra-scleral pass of the same suture and then the entire suture is hidden and not visible under the conjunctiva and is therefore protected from erosion.
