This patient has a relatively opaque cataract with a pre-operative vision of only counting fingers. When we go to perform hydro-dissection, it seems to go well without issues. But for some reason, the nucleus will just not rotate. We try again and again, but no luck. The zonular support and capsular status seems normal, so a decision was made to perform phaco chop within the capsular bag. Once the nucleus was removed, it became clear the reason why nucleus rotation was not possible.
