2961: Aphakic for three months
This case demonstrates a challenging scenario where a patient has been left aphakic for three months following a complicated primary surgery where sectoral zonular loss was noted. Over this twelve-week period, the remaining capsular bag has undergone extensive fibrosis and severe contraction, exacerbated by significant underlying zonulopathy. Our operating guest surgeon demonstrates great skill by successfully re-opening the fibrosed, fused capsular bag using meticulous viscodissection. Once the bag is re-created, a Cionni CTR is inserted and sutured to the sclera via a Hoffman pocket to provide good long term support. With the bag stabilized, a new three-piece IOL is safely placed entirely within the capsular bag. This video beautifully illustrates the critical decision-making and precise tissue manipulation required to salvage a native capsular bag that most surgeons would have abandoned. It is an outstanding example of advanced anterior segment reconstruction.
