Pterygium surgery has evolved significantly, moving away from high-recurrence bare sclera techniques toward the gold standard of conjunctival autograft. In this case, we demonstrate how to simplify the procedure by utilizing fibrin glue instead of traditional sutures. The use of bioadhesive not only reduces surgical time but also dramatically improves postoperative patient comfort by eliminating the foreign body sensation associated with nylon or vicryl sutures. We highlight the critical steps for success: meticulous excision of the pterygium head, thorough smoothing of the underlying bed, and the harvest of a thin, tenon-free limbal graft. Ensuring the proper orientation of the graft is vital for long-term success and preventing recurrence. By transitioning to a sutureless technique, surgeons can provide a more aesthetic result with a faster recovery profile. Beautiful case!
