2800: Quiz: You think this is good, but it’s not

Let’s start the new year with a quiz. Look carefully at the title photo and tell me what the issue is. This resident case seemed totally normal and at the end of the case both the resident and the professor/consultant thought that it was good. But it is not. Look and see if you can figure out what the problem is. Once I show you, it will be obvious. This is a tough one and I suspect that 80% of CataractCoach viewers will not answer correctly. I can tell you that it is not retained lens material; the zonular support is good; the posterior capsule is intact — so what is the issue?
