This is another case featuring the tilt and tumble technique described years ago by Dr Dick Lindstrom. This patient has a cataract which is severe and limits the vision to 20/200 or worse because of very opaque, thick cortical spokes right in the visual axis. The lens nucleus, however, is relatively soft and mushy so performing nucleo-fractis in the capsular bag will not be easy. A better choice for this surgeon is to tilt the nucleus out of the capsular bag and tumble it into the phaco tip. Great case by a former resident of mine 🙂
