1285: remove the central capsule wrinkle

In very myopic eyes, the capsular bag is actually bigger, the same way that the corneal white to white diameter is bigger. This means that we can get a wrinkle in the central posterior capsule when we insert the three-piece IOL into the capsular bag. If you leave it like this then you will get a Maddox rod effect where the patient will notice streaks at a particular angle emanating from oncoming light sources. The treatment is surprisingly easy and you can use this pearl to remove the wrinkle right there on the OR table at the end of the case. Your patients will thank you for this.
