1254: intra-nuclear stone formation

This patient has suffered from a dense white cataract for many years. During that time the lens cortex became liquefied and then presumably absorbed like a morgagnian cataract. The remaining nucleus also developed two spots of dense calcification which produces two stones which are impermeable to ultrasonic phaco energy. This is a rare occurrence and happens only once in every few thousand cases. I have never seen in personally, but I learned from the experience of our guest surgeon Dr Pradip Mohanta from West Bengal, India. During the course of his 30 year career and over tens of thousands of cataract surgeries, he has only seen this condition a couple of times. Fortunately, his most recent case was captured on video and we present it here for your learning.
