Quiz: Why is this phaco chop going to fail?

Look at this photo carefully. We can tell that this phaco chop will fail and the nucleus will not be effectively split into halves. Here is why.

The nucleus is partially prolapsed out of the capsular bag using hydro-dissection.
If we place the phaco probe and the chopper very close to each other then there is less of cleavage plane created and the chop will likely fail.
By increasing the distance between the phaco tip and the chopper, we are able to create a larger cleavage plane and propagate the chop through the nucleus, thereby creating two separated halves.
