With a posterior polar cataract, the posterior capsule can be weak, fragile, or even focally absent and this poses challenges during cataract surgery. The main risk is posterior capsule rupture which has been reported in about 30% of cases, but we can lessen this risk substantially by certain techniques.
The main issue is to avoid hydro-dissection and just perform hydro-delineation instead. Once the endo-nucleus is removed, we can use visco-dissection to remove the epi-nuclear shell and the cortex.
