Misloaded IOL leads to fractures of the optic

In this case, an ophthalmologist in training is performing this surgery during his residency program. The case looks great, but when it comes to loading the IOL, he makes the mistake of grabbing the center of the trailing haptic and pushing it into the injector. This causes the trailing haptic to fold over and that causes increased thickness of lens material as it gets compressed in the injector tip.

The trailing haptic should have been grabbed by the very tip so that it would be less bulky and allow compression without damaging the optic. This is another example of why you must follow the manufacturer’s directions to the letter.
